Programs

We provide a supervised and supportive setting, onsite behavioral health services and integrated health and wellness programming within a warm and welcoming environment. Treatment plans are tailored to each individual and designed to promote recovery, resilience and independence. Residents learn how to manage their mental health symptoms, and to live independently, enabling them to meet their personal goals of education, employment, permanent housing, and healthy relationships.

Residential Program

Gray Haven is a licensed Adult Residential Facility and plans to develop additional transitional independent living options.

The Residence

  • Currently licensed for 6 residents
  • 24-hour supervision, 7 days a week
  • Nutritious meals accommodating special diets as needed
  • Housekeeping, laundry, repairs and maintenance
  • Planned social and recreational skill-building activities
  • Onsite mental health and other supportive services
  • Assistance with life skills such as job placement, budgeting, appointments, transportation, and interpersonal communication

Supportive Services

Behavioral health and other supportive services are a critical component of our program. Our staff has significant expertise working with individuals with mental health challenges who were formerly incarcerated and helping them to safely transition back to the community.

Comprehensive psychosocial assessment including:  mental and physical health, medication use, substance use history, criminal justice involvement, vocational and educational potential, psychosocial history, and social and family supports.

Development of Individualized Needs and Services Plan incorporating resident and family/caregiver insights and contributions.

Evidence based mental health treatment that integrates individual and group therapies with forensic psychiatry and medication management.

Case management and service coordination, including assistance navigating community resources and social service agencies.

Life skills training on medication management, self-care, relationships, and job skills as well as peer counseling and other recovery supports.

Wellness Program

Gray Haven offers an assortment of social, educational, and recreational indoor and outdoor activities. These activities present opportunities for skill building, bonding, and just having fun. Onsite facilities include a pool and recreation room, groups, and classes that include yoga, 12-step, weight training, financial literacy, anger management, art therapy, and more.

A typical day includes meals, medication, reentry classes, meetings, outdoor activity, ADL tasks, art/music activity, evidence-based individual and group therapy, games, and television or movies. These activities provide consistent and supported opportunities to learn and practice mental heath symptom management and improve social skills in a safe, home-like environment
